Traptox — short for trapezius tox — is a neuromodulator treatment injected directly into the trapezius, the large muscle that runs across the back of your shoulders and up into your neck. Relaxing this muscle can soften a bulky or “boxy” shoulder line, giving the neck and shoulders a longer, more sloped appearance, and can also ease tension that builds up in the area.
You may hear this treatment called “Barbie Tox” — an informal nickname that’s become common in the industry for its slimming effect on the shoulder line, not a distinct treatment. It’s the same tox injected into a different muscle, chosen because of what many patients are looking to change.
As with any tox treatment, results aren’t immediate — it takes time to see the muscle respond and settle. The dose and exact injection points are mapped out for your anatomy and goals at a consultation, not applied as a standard package.

Traptox is generally sought out for a couple of specific, related concerns.

Softens a prominent or “boxy” trapezius for a longer, more sloped neck-to-shoulder line.

Relaxing the trapezius can also ease the tightness that builds up in the upper shoulders and neck.
Here’s the honest rundown of a Traptox appointment, from treatment to results.
A series of small injections are placed across the trapezius muscle with a fine needle. Appointments are quick, with no cutting and no anesthesia required — some mild, brief redness or swelling right at the injection sites is normal.
Session length: Quick, in-office appointmentResults aren’t instant — the muscle gradually relaxes over the following one to two weeks as the effect settles in, softening the shoulder line and easing tension along the way.
Onset: Gradual, over one to two weeksThere’s no real downtime — most patients return to their day right after. The effect is temporary and gradually fades over time, which is why maintenance sessions are part of an ongoing plan for most patients.
Recovery: Minimal to noneYes — it’s the same tox treatment used for facial lines, injected into the trapezius muscle instead. The technique and material are the same; the difference is the muscle it’s targeting and the effect it produces.
“Barbie Tox” is just an informal nickname people use for Traptox, referring to the longer, more sloped neck-to-shoulder line it can create. It’s not a different treatment — same injection, same muscle.
Results aren’t immediate — the muscle gradually relaxes over the following one to two weeks as the full effect settles in.
The effect is temporary and fades gradually over time. Exactly how long it lasts varies by person and dose, which is why we talk through a maintenance schedule that fits you at your consultation.
Most patients describe it as a quick pinch rather than significant pain. There’s virtually no downtime — some mild, brief redness or swelling right at the injection sites is normal, but most patients return to their day immediately after.
The dose is chosen carefully so the trapezius still functions normally for everyday movement and exercise — we’ll talk through your activity level and goals at your consultation to plan accordingly.
